Why a Foundation?
The Big Stone Health Care Foundation was
established in 1990 when concerned residents recognized that additional
resources were needed not only to maintain existing local health care but also
to ensure the continuation of this quality health care for the the future
generations of our community.
As health care recipients, we all demand and
deserve equipment and facilities that can provide quality basic health
care. We also expect highly trained and competent health care
professionals. However, changes in the health care market have meant that
fees for services and government reimbursement do not always cover the necessary
costs of providing health care.
The Big Stone Health Care Foundation provides a
vehicle to meet these challenges now and in the future. It also provides
an opportunity for you to invest in your community with a tax0deductible
contribution.
Your
Involvement...
When you decide to make an annual gift, a
memorial gift, a life income gift, a bequest or any other type of contribution,
you are making a most important decision. You are choosing to support and
enhance health care services, something that touches each of us at some point in

"TREE
OF LIFE"
The "Tree of Life" program recognizes
the generosity of our donors. It is carved of beautiful oak wood, and is
located in the lobby of the Ortonville Hospital.
The unselfish gifts which are given by friends,
families and former patients are life leaves and branches on a tree. They
are vital for life and growth.
Recognitions include:

Building Project |
| Ortonville Area
Health Services is Building for the Future by constructing a new
Outpatient Service Center. The Big Stone Health Care Foundation and
OAHS are conducting a capital campaign with a goal of $500,000 to help
with the $5 million project.
The Outpatient Service Center is being built in response
to an increase in outpatient services over the years and an aging physical
plant. It will allow us to provide even better services for our
changing health care needs.
Big Stone Health Care Foundation has committed $50,000 a
year over 5 years for a total of $250,000 for needed equipment.

| Northside
Medical Center |
|
This space will include an
area for the 5 physicians and 2 nurse practitioners we currently have on
staff. We will also have enough space to recruit an additional full
time provider. A central registration area will facilitate patient
traffic.

| Specialist
Physician Outreach Area |
|
This area will be designed for
the specialist to see their patients in a constant designated place.
It will allow the visiting physicians and local physicians to practice
side by side.

| Chemotherapy
Center |
|
1,200 square feet will be
dedicated to provide space for families and friends to support the patient
while they are receiving treatment. The new space will be private
and very comfortable.

| Physical
Therapy/Rehabilitation |
|
Occupational Therapy and
Occupational Medicine, Cardiac Rehabilitation, Physical Therapy and an
exercise area will be located on the first floor. Other services
will be able to be expanded.

donation by etching in granite your name, business name, organization or
the name of someone you wish to honor. Donations levels will be
distinctly recognized at these levels:

| How can I make
a gift to the Building for the Future Campaign?
Gifts may be given as unrestricted gifts to be used where
needed most, or you may choose to make a restricted gift to benefit a
specific department, program or service.
You can also support this
project by participating in the Oak Tree
Classic in the 11th Annual Golf Tournament on September 7, 2002, at
the Ortonville Golf Course.
